The CapsulCard™ - Durable Medication List

What exactly is a CapsulCard?
The CapsulCard™ is a durable wallet sized medication list on which a person's health care providers' names, pharmacy name, and medication information is printed on the surface. The goal of the medication list is to prevent medication errors and allow clinical pharmacists to answer your medicine questions.



A maximum of 14 medications can fit on one card (front/back);
if your medication information don't fit, we'll put remaining medication information on extra cards for FREE
