42% of all Americans have taken a prescription medication. Elderly patients account for >50% of all prescription drug use1. Based on an Institute of Medicine report, for every 1 medication prescribed, there is at least 1 associated adverse drug event. The prevention of medication errors has become a national public health priority.

Patients are routinely prescribed multiple medications by multiple physicians. In practice, pertinent medical consult reports are often not received by the patient’s primary care doctor in a timely manner.
Medical Records
Few clinics have electronic medical records (<24%)2, prescribe electronically, or are linked to pharmacy systems for medication profiles. Unfortunately, this leads to instances where a health care provider may have to make a medical decision on which medication to prescribe or change without complete knowledge of the patient’s medication history.

Health care providers do not often have the time in a 20 minute patient visit, to physically review a patient’s medications or call the pharmacy each time to see which prescriptions have been actually picked up3, 4. Keeping track of a patient’s medication profile becomes even more difficult for elderly patients who are often on several medications. Due to age-related cognitive impairment, many of these patients have difficulty remembering which medications they are taking at home. A physician’s medical record list can be inaccurate if a patient is not taking certain medications due to financial reasons or is taking multiple over the counter products.
